Consumer Rights & Records Access

(Required by Texas Law – Health & Safety Code §181.105)

Access to Your Health Care Records

As a client of this practice, you have the right to request access to or copies of your health care records that I maintain.

To request your records:

  • Please submit a written request by email or through the secure client portal

  • Requests should include your full name, date of birth, and signature

  • Records will be provided in accordance with applicable Texas and federal laws, including HIPAA

  • Reasonable fees may apply as permitted by law

Filing a Complaint With the Licensing Council

If you have concerns regarding my professional conduct, you may file a complaint with the appropriate Texas licensing authority.

Filing a Consumer Complaint With the Texas Attorney General

You may also file a consumer complaint with the Office of the Texas Attorney General if you believe your rights as a consumer have been violated.
